You're at your desk when a deep ache starts in one buttock and slowly creeps down the back of your leg. Is it sciatica? Or something tighter in the hip, like piriformis syndrome?

Patients ask us this all the time, and it's a fair question. The two feel almost identical. But one usually starts in the spine and the other in a muscle, and the treatment isn't the same. Here's how Dr. Shrikant Dalal, spine surgeon at Orthos Centre in Baner, helps people tell them apart.

What Is Sciatica? Causes and Symptoms

Sciatica isn't a diagnosis on its own. It describes pain that travels along the sciatic nerve, from the lower back through the buttock and down the leg.

The usual cause is a nerve root in the lower spine being squeezed, most often by a slipped disc or a narrowed spinal canal. That's why sciatica often comes with back pain, tingling, or a feeling of weakness in the leg.

What Is Piriformis Syndrome? Causes and Symptoms

The piriformis is a small muscle deep in the buttock that helps rotate your hip. The sciatic nerve passes right beside it, and in some people through it. When the muscle tightens or goes into spasm, it can irritate the nerve.

The typical picture is a deep ache in the buttock that gets worse after long sitting or driving, or when climbing stairs. Doctors still debate how common it really is, and it's often diagnosed only after spine causes have been ruled out.

Red Flags: When Sciatica Pain Needs Urgent Care

Go to an emergency department if you have any of these:

  • Numbness around the groin or inner thighs
  • Difficulty passing urine or stools, or loss of control
  • Weakness in both legs
  • A foot that drags or slaps when you walk
  • Back pain with fever, or after a fall or injury

These can point to serious nerve compression, such as cauda equina syndrome, which needs urgent treatment.

How a Spine Specialist in Pune Diagnoses the Cause

At the clinic, the first step is a proper history and physical examination. Dr. Dalal checks your reflexes, muscle power, sensation and nerve tension tests, and also examines the hip and buttock to look for piriformis tenderness.

Sciatica Treatment in Pune: Most People Don't Need Surgery

Most sciatica improves with time and the right care: guided physiotherapy, movement modification and medicines prescribed by your doctor. Piriformis syndrome is usually treated with targeted stretching and soft-tissue work, and sometimes an injection.

Surgery becomes a conversation when leg pain persists for weeks despite treatment, or when weakness is getting worse. Even then, many disc problems can be treated with minimally invasive spine surgery rather than open surgery.

What we advise against: weeks of bed rest, long-term painkillers without supervision, and aggressive stretching that makes the pain worse.
